You must provide your contact information and consent to the disclosure requirement before submitting the report.
Arizona Disclosure Requirement
Under Arizona law (A.R.S. §33-1242 and A.R.S. §33-1803), any complaint filed with the Association will not be kept anonymous. Individuals reporting a violation must provide specific details, including the date the violation was observed and their first and last names. Please note that your name will be disclosed to the violator upon request.
